1. Acceptance of Terms

By accessing and using the Czech Rehabilitation Hospital online consultation request platform (the "Platform"), you accept and agree to be bound by these Terms and Conditions. If you do not agree to these terms, please do not use the Platform.

2. Platform Purpose

This Platform is provided solely for the purpose of:

• Submitting consultation requests for surgical procedures
• Scheduling appointments with our medical staff
• Providing preliminary medical information for consultation preparation

This Platform does NOT provide medical advice, diagnosis, or treatment. For medical emergencies, call emergency services immediately.

3. Eligibility

To use this Platform, you must:

• Be at least 18 years of age, OR
• Have parental/guardian consent if submitting on behalf of a minor
• Provide accurate and truthful information
• Have legal capacity to enter into this agreement

4. Your Responsibilities

When using the Platform, you agree to:

• Provide accurate, current, and complete information
• Update your information if it changes
• Respond to our communications regarding your consultation request
• Attend scheduled appointments or notify us of cancellations
• Not misuse the Platform or submit false information
• Not upload malicious files or attempt to harm the system

6. Consultation Process

After submitting a consultation request:

1. Our administrative staff will review your submission (typically within 1-3 business days)
2. You will be contacted via phone, WhatsApp, or email to schedule an appointment
3. You must attend an in-person consultation for medical evaluation
4. Treatment decisions will be made only after proper medical examination
5. Surgical procedures require informed consent and additional documentation

7. Data Protection & Privacy

Your personal and health information is protected in accordance with our Privacy Policy and UAE Federal Decree-Law No. 45 of 2021. By using the Platform, you consent to the collection, use, and processing of your information as described in the Privacy Policy.

8. Intellectual Property

All content on this Platform, including text, graphics, logos, and software, is the property of Czech Rehabilitation Hospital or its licensors and is protected by UAE and international copyright laws. You may not reproduce, distribute, or create derivative works without written permission.

9. Limitation of Liability

To the fullest extent permitted by UAE law:

• The Hospital is not liable for technical issues, delays, or unavailability of the Platform
• We are not liable for any loss or damage arising from your use of the Platform
• We do not guarantee uninterrupted or error-free operation
• Medical malpractice claims are governed by separate medical services agreements and UAE healthcare laws

10. Cancellations & No-Shows

If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance. Repeated no-shows may result in restrictions on future consultation requests.

11. Fees & Payment

Submitting a consultation request through this Platform is free. Consultation fees, surgical fees, and other medical costs will be discussed during your in-person appointment. Payment terms will be provided separately.

12. Termination

We reserve the right to suspend or terminate your access to the Platform at any time, without notice, for:

• Violation of these Terms & Conditions
• Providing false or misleading information
• Abusive or threatening behavior toward staff
• Repeated no-shows without notification
• Any other reason at our sole discretion

13. Governing Law & Jurisdiction

These Terms & Conditions are governed by and construed in accordance with the laws of the United Arab Emirates and the Emirate of Abu Dhabi. Any disputes arising from these terms or your use of the Platform shall be subject to the exclusive jurisdiction of the courts of Abu Dhabi, UAE.

14. Changes to Terms

We reserve the right to modify these Terms & Conditions at any time. Changes will be effective immediately upon posting on the Platform. Your continued use of the Platform after changes are posted constitutes your acceptance of the modified terms.

15. Severability

If any provision of these Terms & Conditions is found to be unenforceable or invalid under UAE law, that provision will be limited or eliminated to the minimum extent necessary, and the remaining provisions will remain in full force and effect.
